Counties lagging in SHA enrollment

The transition from the National Health Insurance Fund (NHIF) to the Social Health Authority (SHA) stands as a fundamental component of Kenya’s universal health coverage initiative.

Yet, recent figures from the Kenya National Bureau of Statistics 2026 Economic Survey reveal considerable unevenness in regional participation.

Although nationwide registration progresses, ten countiesmainly located in the Frontier Districts and Coastal areasare showing slower adoption rates.

Isiolo demonstrates minimal participation with only 46,968 sign-ups, with Lamu and Tana River following closely behind.

The statistics indicate a pattern where large or historically disadvantaged counties, including Marsabit, Samburu, and Turkana, encounter greater difficulties implementing the updated health system.

Even areas such as Taita Taveta and Tharaka-Nithi maintain figures under the 180,000 threshold.

These numbers emphasize the immediate requirement for focused public awareness campaigns and distributed registration centers to prevent the shift to SHA from excluding the nation’s most at-risk communities.
