The shift from the National Health Insurance Fund (NHIF) to the Social Health Authority (SHA) on October 1, 2024, represented a significant transformation in Kenya’s healthcare payment system. According to the latest Kenya National Bureau of Statistics 2026 Economic Survey, Nairobi City leads the country in SHA enrollment, boasting more than 1.6 million registered participants. This prominent position demonstrates the effect of focused urban promotion and substantial employment concentration in the metropolitan area. The Central and Rift Valley regions also demonstrate strong participation, with Kiambu and Nakuru trailing Nairobi with 832,307 and 730,260 registrations, respectively. The Coastal and Western regions are represented by Mombasa and Kakamega, both exceeding the 700,000 registration threshold. Additional high-performing counties include Bungoma, Kisumu, Machakos, Homa Bay, and Kisii. These statistics indicate consistent nationwide progress toward universal healthcare coverage, showing increasing public trust in the digital transformation of health services and the organized registration of beneficiaries across various regional areas.
